Ultraweb™ Joined Forms

These label forms are a Stat Products, Inc. exclusive! They are designed to maximize your dot-matrix printer, and put an end to the separate label, receipt, and hard copy forms by combining them all into one. Our Ultraweb™ forms are available in two sizes and styles:

Ultraweb 3800 - JW

The Ultraweb JW is a combination paper and pressure sensitive joined form which features both receipt and hard copy areas.

Ultraweb 3700 - JX

The Ultraweb JX is a combination paper and pressure sensitive joined form without hard copy, and it repositions the receipt and patient counseling to the right hand side of the form.

Bond Forms with a Pressure-Sensitive Label

Our NP-1, a variation of our popular N format, features heavyweight paper with a smudge proof pressure-sensitive label with an E-Z peel tab affixed to the form. An extra pressure-sensitive label can be added to the right of the prescription label for an additional charge. This form is also available without a pressure-sensitive label (NP) resulting in a taller prescription label capable of holding ten lines of computer generated copy in lieu of nine.


Bond Forms

N - Bond Form

For the dot matrix printers, our standard style N bond form has been the most popular style since its development many years ago, and is still a best seller today. It is strictly a paper form, and features hard copy, receipt, patient counseling, and prescription areas.


Pressure -Sensitive Labels

A pressure-sensitive label is most commonly referred to as a "sticky label". Each section may be peeled away from the label liner and applied to a bottle or other surface for permanent application. Although used in typewriters many years ago, pressure-sensitive labels are now used in dot-matrix printers. These labels are available in a vast variety of styles; with a choice of smudge proof litho (SPL) or Cast-coated hi-gloss (KK) stocks.

A nursing home label is a regular prescription label used with medication cards for reordering drugs and updating Medication Administration Records (M.A.R.). This type of label has a distinct advantage over the pressure-sensitive label because it features one copy directly on top of the other and provides for duplicate information. The top copy is placed on the medication card and the bottom copy is used for reordering and monthly updating. This format is especially helpful to nurses who typically hand write information every month and are concerned over possible transcription errors and legible copy.
