We have 4 1/8" x 5 1/4" custom imprinted and stock prescription blanks available. The minimum order for custom imprinting is 4,000 Rx blanks (40 pads of 100 sheets per pad). We stock prescription blanks which conform to the Massachusetts State Pharmacy Board regulations. This MA-1 style blank is printed in blue ink on white bond paper. Minimum of 1,000 blanks (10 pads). Our stock receipt pads feature multiple label areas to affix your computer generated receipts. Available in the four different formats shown below, you can order as little as 1,000 receipts. For use with medication/blister cards in the nursing home, our drug reorder books are available with and without a reorder tab. Each page features an orderly log of dispensed medications. A nurse may either transcribe the necessary information or affix a computer generated Rx label to the log, thus eliminating the potential for transcription errors and provide for a more legible log. Our drug reorder book with reorder tab has a removable section featuring either the Rx number or the Rx number, patient name, and drug name which can be conveniently forwarded to the pharmacy as needed. Physician's Orders / M.A.R. Form
Keep track of any changes in medication with this handy all-in-one notebook-style form. Part 1 (chart copy) and Part 2 (pharmacy copy) are carbonless and contain duplicate information. The following popular styles of labels are available on an ongoing basis, should the need arise to use blank labels until your printed order is manufactured. All stocked pressure-sensitive labels will be available only with a smudgeproof litho facesheet, unless otherwise noted. A minimum quantity requirement is 1,000 labels. If your order is received by 3:00pm Eastern Standard Time, we can ship the same day via RPS or UPS.
